Due to the potential risk of infection from the Novel Corona virus (COVID-19), all personnel visiting Olympic Promoter for either service work or other relevant issues, are required to answer and sign the following questionnaire.

The given information will be treated as confidential, however you must consent to the information being made available to health authorities in the event of a suspected case of this virus on board the vessel.

When joining the vessel, we refer to the Olympic procedure for handling communicable diseases on Olympic vessels for more information.

Face mask mandatory first 5 days onboard if not possible to hold good distance.
Not allowed to use gym first 5 days onboard. (Risk assessment can be made to allow 1-2 persons at a time in this period)
Minimal use of mess and day rooms the first 5 days onboard.
Keep social distance of at least 2 meter if possible.
Rapid Antigen test to be performed within 24 hours of arrival the vessel.
